Vaccine Campaign for 12 to 17 Year Olds

The COVID19 vaccine is available for children aged 12 to 17. You can now make an appointment for your child through the Province’s “ClicSanté” website.


For families unable to bring their children to appointments offered through ClicSanté, the government has asked our high schools to organize vaccine “field trips” to local vaccination centres. See below for more information.

If your child is 12 years old and in elementary school, we will communicate with you shortly.

CVR & HSB Vaccine Appointments

If you choose to have your child vaccinated through the vaccine “field trips” organized by CVR & HSB, there are a few steps to follow. Documents and links are included below.

  • Read the info provided in the brochure
  • Make your child’s appointment using the link below; register with your child’s school.
  • Complete and return the Government’s Consent for Vaccination Against COVID19 for Users Under the Age of 14 to your child’s school.
    • REQUIRED if your child is 12 or 13
    • Print, complete and return by June 4, 2021, or your child will not be able to receive the vaccination.
  • Complete and return the NFSB’s Transportation Consent Form to your child’s school.
    • REQUIRED for ALL AGES, 12 TO 17.
    • Print, complete and return by June 4, 2021, or your child will not be able to get on the vaccine bus 😐
